Don't use an incandescent bulb, they put out some heat but the light they throw is garbage. I personally haven't used UV yet, I just use full spectrum tubes and CFLs for basking. Exo terra makes fixtures to fit on top of their cages, but a 12" fixture only holds one CFL bulb. Here's a link to the fixture

The problem with the UV bulbs is that they don't throw off too much light and the color temperatures aren't advertised so I can't tell you if they're any good beyond putting out some uv. If you want to use UV then you could get the 5.0 and just put a could 5000k CFL in a clamp fixture and point it at the viv.
Right now I'm only about 6 weeks deep with my Klemmeri so I can't give to0 much long term advice but I'm still not sold on the necessity of UV bulbs. The whole thing doesn't seem all that scientific, especially considering that the companies that make these bulbs also make plastic plants, hot rocks, and a bunch of other relics from the 90's that nobody who knows what he's doing bothers with anymore.
